What is the engine speed when this happens?
Any mods to engine?
Stock converter or higher stall?

Yes but what rpm do you call tick-over?
Measured with an accurate gauge and NOT a factory tach of course.

Should be around 800-850 maybe even lower on a bog stock motor, so if your tick over is at 1200 its to high.
What does it idle at when in gear?

If I ran my tickover that high it would laso bang into gear. at cold, lump 850rpm it drops into gear seamlessley.
Tickover too high. Have you checked fluid levels?
