I had the exact problem with mine and it was the 2 x 11mm bolts that clamped the column housing ( 19 14 21 ) to the 2nd housing ( 19 46 b ) the two bolts slotted through the upper sleeve into retainers in the 2nd housing - they had come lose and fallen out of their holes and were rattling around in the lower housing. I refitted them and twisted the upper housing to the right and it locked into position. I tightened the 11mm bolts and the column and housing tightened up solid.
